I may use or disclose your PHI for:
Treatment: Coordination of your care.
Payment: Billing and insurance (if relevant).
Healthcare Operations: Quality improvement, compliance, scheduling, and administration.
Other disclosures may occur when required by law—for example, in emergencies, court orders, or to prevent a serious threat to health or safety.
4. Uses and Disclosures That Require Your Authorization
I will not disclose your PHI for marketing, fundraising, or the sale of information.
Any non-routine sharing requires your written permission, and you may revoke that permission at any time.

I securely store your PHI in SimplePractice, a HIPAA-compliant EHR platform that protects client data using encryption, access controls, and secure cloud architecture.
